I am wondering if certain diseases (like stroke) are more likely with decreased access to care? Is that why there are differences across states?

That is a good question, maybe because chronic disease (i.e. cardiovascular disease) have a need for long term access to healthcare providers for both detection and treatment.

What do you think about the impact of access of care to more acute diseases?
