CAD differences between men and women exist for because of genetic and environmental factors, which is true for all diseases. Which makes my point, I think the different responses to CAD treatments is based on individual genetic and environmental factors. I am not sure if gender plays a major role in the differences but I must admit I am not an expert in the area.

Does anyone know of any statistically significant biological differences between the genders that can explain differences in the manifestations and treatment of CAD?

There are genetic differences between races and gender which should be considered when doing a differential diagnosis. It is what the author called "using your head".

But you make a good point, evidence based medicine should be based on statistically rigorous studies indicating the biological differences between populations for each disease.

How much data is available documenting the differences between populations for heart disease, cancer and other common diseases?

I believe there is plenty of data available regarding differences in disease manifestations in different populations.

The more important point in my opinion is identiify what treatments are more likely to work in a particular patient.

I think what would be interesting is to identify the interplay between genetic and environmental factors in different populations. Have there been published studies in this area?

Some good questions have been brought up, I think genomics studies could be used to address some of them.
